Proposals due January 6, 2012

To submit a presentation proposal, please provide typewritten responses to the following questions:

1. Title of presentation

2. Name, address, phone, and e-mail of presenter(s): List contact person first

3. Description: A full description of your presentation (75-150 words)

4. List objectives that your presentation will fulfill

5. Abstract: A brief summary of the presentation for use in the conference program (25-50 words)

6. Vita or resume of presenter(s): This must include education, certificates, work experience, publications, and presentations (This is not needed if the presenter has sent this information within the last 2 years)

7. Length of presentation: Please specify whether your presentation requires 60 or 90 minutes (Please note: times may need to be adjusted due to conference scheduling needs)

8. Target Audience: Student, Professional, Experienced Professionals

9. List audio, visual, and/or music instrument needs: Available equipment includes: overhead projectors, Slide projectors, VCR/Monitor, Audio Equipment, Instruments (Please specify the type and number of instruments required)

10. Other special arrangements or requirements: Audience size or type, participant requirements, space needs, internet access etc.
