The student (or parent for PLUS Loan Requests) is considered the "owner" of the form and must
complete and sign the form first. Prior to completing their section of
the form, the student will be asked to provide contact information for the
other signers (parent or spouse for example). Upon the student completing and
electronically signing their section of the form, an email will then be sent
to the other signer requesting they complete their section.
The other signer will log in to Dynamic Forms and will be taken
directly to the form. Please note that they will need to create a Dynamic
Forms account if they do not already have one. The other signer will be able
to see what the student entered but will not be able to edit that information.
The other signer only has access to edit the information within their section.
The multi-signature process relies on email notifications.
System generated emails are sent from
notify@nelnet.net. Once the student has completed and electronically
signed their section of the form, an email will be sent at that time to all
other form participants. The text of this email follows:

You are receiving this e-mail because {Student Name} needs your
help in completing the {Form Name}. Please click the link below to complete
your section of the form.
You will be asked to electronically sign the form.
If you already have a Dynamic Forms account, please log
directly in by entering your Username and Password. Otherwise, please create
an account by clicking on the Create an account link.
You can use the Forgot your username and Forgot your password
links if you are unsure about your Dynamic Forms credentials.
Click here to complete your section of the form.
Once a co-signer has completed and signed their section of the
form, the form owner (student) will receive a confirmation email.
Cc: Subject: Dynamic Forms: Co-Signature update
{Co-signature name} has completed their portion of the {form
name} form.
