What is the best format?

Your assessment plan can take any format that you feel is most appropriate for your program(s). A Microsoft Word document that provides your plan in narrative form is fine, as long as you include the “Essential Elements”. Again, your assessment plan does not have to be a long, extensive document, however, it must communicate the essential elements so that it is an effective “roadmap” to conducting assessment.

A downloadable template in two parts (Microsoft Word) is available for your use. The first part is a narrative portion that includes program mission, goals and objectives. The second part is a matrix that incorporates all the other essential elements. The information that you seen in the columns of the matrix template are there for your guidance, and can be removed as you use the template to construct your own assessment plan. Add pages as necessary as you use the template.
